Course Summary: Orthodontists and surgeons often treat patients with impacted maxillary canines. These teeth may be positioned on the labial or in the palate. The orthodontic mechanics to erupt impacted canines is not complicated, but the final esthetic result may be compromised if the surgical uncovering or the orthodontic eruption is performed improperly. The same surgical procedure should not be used for all impacted canines. The surgeon should select the appropriate method based upon specific criteria. In addition, the timing of uncovering impacted canines should vary depending upon the location of the impacted tooth. Some impacted canines will erupt spontaneously if uncovered before orthodontic bracketing, while others should be uncovered after initial orthodontic alignment. In some cases, a midalveolar impaction may cause root resorption of an adjacent maxillary lateral incisor. Making the correct decision at the appropriate time in this situation is of paramount importance, in order to salvage both the lateral and the canine. This presentation will discuss the many ramifications that are encountered during the surgical and orthodontic management of impacted canines.
